This form is to be filled out by the proposed God[parent or Sponsor and taken to that person's parish to be affirmed and officially stamped. Then it may be turned in to Faith Formation at St. Joseph's.

The custom of taking on a saint’s name at Baptism or Confirmation is still regularly observed at St. Joseph's. Please indicate on this Confirmation - Saint Name Form 2025-2026 FORM whether you wish to do so!
